Examples of preauthorization treatments include, but are not limited to:
- Oral Surgery (Implants/Complex Extractions)
- Advanced Restorations (Crowns)
- Complex Root Canal Therapy
- Advanced Gum Disease Treatments/Surgeries
- Complete and Partial Dentures
- Oral Surgery Complex Extractions, Implants)
- Specific Types of Anesthesia

You may be eligible if:
- Your household income is less than $90,000 annually
- You don’t have dental insurance
- You are over age 65 or under age 18
